WHITCHURCH residents have met with local health experts to find out more about the Shropshire Care Closer to Home Programme.

A drop-in session was held at the Whitchurch Community Hospital, hosted by members of the Shropshire Clinical Commissioning Group (CCG).

As part of a series of pop-up events hosted by the team, the session provided an opportunity to hear the people of Whitchurch’s thoughts on how the new project could best meet people’s needs.

The Shropshire Care Closer to Home Programme is a transformation project aimed to help local people over 65 with a number of health conditions, receive joined-up care and support from the NHS and Shropshire Council in their own home or local community and therefore avoid a hospital stay.

The programme is also about providing health, mental health and social care from organisations working together in the local community as one team. It aims to reach people earlier, and keep people as well as possible and at home for as long as possible.

It hopes to provide one familiar point of contact for the patient and family who will co-ordinate the care and support provided. This means patients, families and carers only have to deal with one person and team.

The next event will take place at the Royal Shrewsbury Hospital restaurant on Wednesday, August 21, between 12.30pm and 3.00pm.
